Credit Application and Policies
The Lab Safety Institute accepts purchase orders from organizations that have completed this credit application and accepted the terms of payment.
Terms of Payment:
1. An invoice will be faxed to the person responsible for preparing payment and a confirmation copy will be mailed. Payment is due upon receipt of the faxed invoice.
2. If full payment is not received within two weeks, a late fee of $25.00 will be applied to the account and an updated invoice will be faxed to the person responsible for preparing payment.
3. Payments not received within one month will be assessed a monthly interest charge of 1.5% of the outstanding balance. An updated invoice will be faxed to the person responsible for preparing payment.
4. In requesting materials please indicate alternate dates and selections. Normally only one item will be sent for each viewing date. Please select the appropriate payment rental fee as follows: (Multiple set video's will be charged per video.)
Members: $12 shipping fee and handling.
Non-Members @ Non-Profit Organizations and Companies: $35.00 shipping fee and handling.
5. The materials should be returned within 30 days and insured for the amount indicated. If not returned within 30 days, a charge of $12 or $35.00 (depending on rental category) per video per month late fee will be assessed.
